Comprehensive Services: Our integrated approach empowers primary care physicians to provide quick triage services, including EKG, Spirometry, audiometry, Holter monitors, 24-hour home sleep studies, X-Rays, Ultrasounds, Electrophysical Therapy, depression screening, alcohol misuse screening, memory testing, diabetes eye exams, fundus camera imaging, and Annual Wellness check-ups.
Patient-Centric Focus: We believe in patient-centric care. We’ve implemented a scribe program to enhance doctor-patient interactions, utilize population health data to address care guideline gaps, and guarantee appointments within 24-48 hours. Each of our locations hosts no more than four providers, ensuring personalized attention.
Proven Growth: Since our establishment in 2010, we’ve expanded from a single clinic to three sites, including two patient-focused locations and one administrative office, with a total of eight providers.
Outstanding Outcomes: Amidst the shift to value-based care, our patients’ outcomes have excelled, with notable reductions in hospital admissions/readmissions, ER/UC visits, and specialty referrals.
